1. Overview
The Care Pathway Builder is a tool for healthcare professionals to design personalized, automated care plans. It enhances patient management by standardizing pathways, automating follow-ups, and setting alerts for key health metrics, making it ideal for long-term or complex care needs.
2. Key features
- Define pathway details like title, duration, involved providers, and locations.
- Use customizable widgets (questions, activities, educational content) to engage patients.
- Schedule appointments and set conditions for dynamic care adjustments.
- Trigger alerts based on metric thresholds to monitor patient health.
- Collect consent and store files directly in the patient record.
3. Create a new care plan
- Access the Care Pathway Builder by clicking on
- Click on "Build a care pathway".
- Fill in general information (title, description, duration).
- Define involved actors (roles like primary physician, nurse, etc.).
- Specify locations where care will be delivered.
4. Building a care plan
- Add necessary elements (widgets and automations) by drag-and-drop. See details in the table below.
- Set periodicity to your elements by clicking on
when configuring them.
- Connect elements to create the flow, reorganize the order as wanted. Create defaults or specific connections depending on answers.
- Define time intervals between actions by using

- Select important data to display and add them in the triage system, by clicking on
. Important metrics will be displayed directly in the patient list view.

5. Setting up alerts
- To add alerts to the care plan, click on
- Define triggering conditions (metric value, periodicity…).
- Specify actions to be taken.
- Assign responsible actors.
- Set resolution timeframes.
- Configure escalations if needed.
6. Publication and assignement
- Review the entire pathway.
- Publish the care plan. A published plan can no longer be modified (only duplicated).
- Assign it to the relevant patients by going to the patient list or patient record.
- A patient can follow multiple careplans simultaneously.
- A care plan can be archived or deleted.
