Add a Single-Choice Question to your care plan

Care pathway builder allows you to add different types of interaction boxes and automation using a drag-and-drop interface.

• Drag and drop the Question widget into the canvas.
• Choose your specified type of question.

A single choice question restricts respondents to selecting only one option from a list.

• Click the "Single choice" element from the different types of questions available.
• Enter your question in the question text box.
• Define the different answers, and their score for each answer. The scoring system is free to your interpretation.

• If needed, set a periodicity for the widget by clicking on . To learn how to set periodicity, go to Set up a periodicity to a widget section.
• Don’t forget to save your new widget by clicking on “Save”.
