Add a Likert Question to your care plan

Care pathway builder allows you to add different types of interaction boxes and automation using a drag-and-drop interface.

• Drag and drop the Question widget into the canvas.
• Choose your specified type of question.

A Likert question measures respondents' attitudes or opinions on a linear scale (e.g., Strongly Disagree to Strongly Agree).

• Click the "Likert question" element from the different types of questions available.
• Enter the statement or question text. You can add multiple questions if needed.
• Define the different answers. You can add or remove answers.
• You can allow multiple answers.
• Set a scoring system to measure out the risk level depending on the patient's answer. The scoring system is free to your interpretation.

• If needed, set a periodicity for the widget by clicking on . To learn how to set periodicity, go to Set up a periodicity to a widget section.
• Don’t forget to save your new widget by clicking on “Save”.
