Editing my patient information

To edit a patient's general information, click on

⚠️ Please ensure that all patient information entered is accurate, complete and up-to-date. Accurate records are essential for effective patient care and treatment planning.

Edit the information needed (e.g., contact, address, emergency contact, coverage, assigned care team..)

1. Use the dropdown arrows to fill in each part required.
2. To associate the patient with another care team, click on “Patients care teams” and select an existing care team. To learn how to create a Care Team, go to the Manage Care Teams section.
3. Once all the fields have been filled, make sure to confirm the changes by clicking “Confirm”.
