On the bottom left side in the Measures section, are the associated Metrics, Questionnaires and Symptoms to the patient.
When patients complete their care plan or answer a questionnaire, they may answer questions that generate metrics values. These values are automatically displayed in this Metrics sub-section.
When a patient completes a questionnaire, a questionnaire score (main score) is assigned at the end based on the answers entered. The main score for this questionnaire is automatically displayed in the
Questionnaire sub-section. If the questionnaire has associated sub-scores then they will be also displayed here. To learn more about the questionnaire scoring system, go to the Questionnaire scoring section.
To manually add a new Metric measure or Questionnaire answers, simply click on . This feature could be used during a consultation with the patient, to manually add answers, without him having to complete a care plan.
1. Add a metric manually
When manually adding a metric to a patient’s record, you can:
- Select an existing one by clicking on the dropdown arrow.
- Create a new one by clicking on
. To learn how to create a new Metric, go to the Manage my metrics section.
- Once the metric has been selected, enter the current value given by the patient.
- To confirm the creation of the metric, click on “Create”.
- The metric with the patient's current value will be shown in the Metrics sub-section.
- To add a new value of the same metric, repeat the process: click on
and select the same metric. Fill in the new value the patient answered.
- The value displayed for the metric is updated as soon as you or the patient enters new data.
1.1. Understanding the symbols used for metrics
The symbols next to the metrics show how the patient's metric measurements are progressing:

defines that the total value of the metric has increased.

defines that the total value of the metric has stabilized.

defines that the total value of the metric has decreased.
1.2. View the evolution of metrics in graphic mode
To view the evolution of the metrics entered, click on the metric result of your choice:
- You can select the period for which you want to see the metrics.
- You can select the aggregation function you want to display:

- Sum: Displays the total number of bottles of wine for the selected period.
- Average: Shows the average number of bottles of wine per day for the selected period.
- Last Value: Displays the number of bottles of wine on the most recent day of the selected period.

2. Add a questionnaire manually
When manually adding a questionnaire to a patient’s record, you can:
- Select an existing one by clicking on the dropdown arrow.
- Create a new one by clicking on
. To learn how to create a new Questionnaire, go to the Manage my questionnaires section.
- To start completing the questionnaire with your patient, click on “Select questionnaire”.
The other way to have a patient answering a questionnaire is to include it in a care plan. To learn how to include a questionnaire in a care plan, go to the Widget - Questionnaire sub-section in the Managing my care plans section.
2.1. View questionnaire results
The results of a questionnaire are colour-coded if you have defined interpretation rules when creating the questionnaire. This defines a risk level for the patient's state of health based on the score obtained in the questionnaire. The risk level is categorised on a scale from 1 (low risk) to 5 (critical risk). To learn more about interpretation rules, go to the Questionnaire scoring section.
For example, a level 1 (low risk) will appear in green, while if no risk level is assigned, the result will appear in black.
To see the answers given by the patient to the questionnaire, you can click on the name of the questionnaire whose answers you want to see.
You will then be able to see the score obtained and the formula used to obtain it.
You will also have access to all the questions and answers given by the patient.
Some questions have scoring systems, so you will be able to see the score obtained by the patient for each individual answer.
