Add a Scale Question to your care plan

Care pathway builder allows you to add different types of interaction boxes and automation using a drag-and-drop interface.

• Drag and drop the Question widget into the canvas.
• Choose your specified type of question.

A scale question allows respondents to rate something on a predefined scale (e.g., 1 to 10).

• Click the "Scale question" element from the different types of questions available.
• Select an existing metric or create a new one by clicking on . To learn how to create a new metric, go to the Manage metrics section.
  
  • • • If the metric already has a number of steps, you will not be able to change it.
      • Optionally, you will be able to add labels on values. To add more labels, click on the arrows.

• • • • If the metric has a scoring system, this will be filled in automatically. You will not be able to change it.

• Enter your question in the question text box.
• Optionally, you can add labels to values and name them.
• Click on “Next”, to see the scoring for this metric.

• The scoring was defined during the metric creation. The chosen metric may not have a scoring system. The scoring system is free to your interpretation.
• You can activate this question for the triage. To learn more about the triage feature, go to the Select Triage measures section.

• If needed, set a periodicity for the widget by clicking on . To learn how to set periodicity, go to Set up a periodicity to a widget section.
• Don’t forget to save your new widget by clicking on “Finish”.
