Add a Questionnaire to your care plan

Care pathway builder allows you to add different types of interaction boxes and automation using a drag-and-drop interface.

• Drag and drop the Questionnaire widget to add it to the canvas.
• Select an existing questionnaire or create a new one by clicking on . To learn how to create a new questionnaire, go to the Questionnaire - General informations section.
• You can activate this question for the triage. To learn more about the triage feature, go to the Select Triage measures section.
• If needed, set a periodicity for the widget by clicking on . To learn how to set periodicity, go to Set up a periodicity to a widget section.
• Don’t forget to save your new widget by clicking on “Save”.

• The patient will have to respond to the questionnaire as a part of his care plan.
  • • • Another way to complete a questionnaire is to complete it manually during a consultation with the patient for example. To learn more, go to Add a questionnaire manually section.

• The results of the questionnaire completed via the care plan will be displayed in the “Measures - Questionnaires” section of the patient record. To learn more about it, go to the View questionnaire results section.
• They will also be visible from the “Care Pathways” section of the patient record, when you click on the current care plan containing the questionnaire. To learn more about it, go to the View my patient's alerts and care pathways section.
