Questionnaire - Add an open question

Questionnaire builder allows you to add different types of questions using a drag-and-drop interface.

An open question allows respondents to provide free-form text answers.

How to add:

• Drag and drop the "Open Question" element to the canvas.

• Add a title to the question.
• Enter your question in the question text box.
• Optionally, you can add limits and a scoring system for the expected words in the patient’s answers. To learn more about these options, go to the Open Question sub-section in the Managing My Care Plan section.
• Don’t forget to save your new question by clicking on “Save”.
