Questionnaire - Add a date question

Questionnaire builder allows you to add different types of questions using a drag-and-drop interface.

A date question is used to find out the date of an event that happened to the patient, for example.

How to add:

• Drag and drop the "Date" element to the canvas.

• Enter your question in the question text box.
• The patient will be able to select a date to respond to the question : Year, month, date, weekday.
• Optionally, you activate the option for the patient to respond with time by clicking on “Include time”: the patient will have to indicate a time in his answer.
• Optionally you can activate the “Cognitive test” mode. To learn more about this mode, go to the Date question sub-section in the Managing My Care Plan section.
• Don’t forget to save your new question by clicking on “Save”.
