1. Understanding the purpose of scoring a questionnaire
The final step in creating your questionnaire is assigning a scoring system, which helps assess the patient's overall condition and identify potential risks based on their responses.
The process of assigning both the main score and sub-scores works in the same way.
- The main score is primarily used to calculate the overall score based on the patient's responses to all questions in the questionnaire. It reflects the patient's overall health status or a specific condition being evaluated by the questionnaire.
- Sub-scores (optional) can be used to calculate the score for specific groups of questions that are related to the same theme or area of concern. You can create multiple sub-scores to track different aspects of the patient's condition.
For some questions, each possible answer is assigned a specific score. The score assigned depends on the patient's response, and these question scores are the building blocks for both the main score and the sub-scores.
To calculate the main score, you must create a formula that combines the patient's results from the various relevant questions. The result of this formula is the Questionnaire Score (main score).
Similarly, to calculate the sub-scores, you can create additional formulas based on the patient’s answers to specific groups of related questions.
Once the main score and sub-scores are determined, you can apply interpretation rules to assess the patient's risk level. These rules help categorize the risk associated with the scores, allowing you to identify areas that may require further attention based on the questionnaire results.
2. Create the Main Score Formula
The formula will calculate the overall score by appropriately combining the patient’s responses.
To build your formula:
- Click on the "+" or "-" symbols and select the questions you want to include in the formula (only questions with a scoring system can be included).
- When building the formula you will have different options, for each question you must select the type of value:

- Raw: This represents the actual answer provided by the patient.
- Score: This is the score calculated based on the patient’s answer.

- You can also add or subtract constant values to your formula by clicking on
and inputting the desired number.
3. Add interpretation rules
Optionally, you can add interpretation rules to your formula, by clicking on , and then on “Add condition”.
This allows you to evaluate the patient’s risk level based on their responses. The interpretation is performed using the result of the main score formula.
The risk level is categorized on a scale from 1 (low risk) to 5 (critical risk). Based on the assigned risk level, the questionnaire result (main score and subscores) will appear in the "Questionnaire Measures" subsection of the patient’s record, color-coded according to the risk level.
For example, a level 1 (low risk) will appear in green, while if no risk level is assigned, the result will appear in black. To learn more about questionnaire interpretation, go to View my patient’s measures section.
Don't forget to save your questionnaire by clicking on “Save”. Please note that once you have saved it, you will not be able to modify it.
